The Division of Family and Children Services would like to provide a uniform standard for IMPACT training materials. To support this effort, we are asking each provider order the estimated number of Participant Guidesand Learning Journals directly from the agency for the current year. We ask that you take the time to complete the survey as this will allow us to send you the accurate number of books. You must specify a contact person and correct mailing address in the survey. There is no cost associated with this request. All surveys must be completed by June 30, 2016. The training materials will be sent directly to your specified address.
