Dear Providers:

The Division of Family and Children Services is continuing our efforts to maintain uniformity in our IMPACT training materials. To support this ongoing effort, we are asking each provider to order the estimated number of Participant Guides and Learning Journals directly from the agency for the current year. We ask that you please complete the survey as this will allow us to send you the accurate number of manuals needed, but not to exceed the maximum allowance. You must specify a contact person and correct mailing address in the survey. There is no cost associated with this request. However, the maximum number for Participant Guides allowed is (50) and the maximum number of Learning Journals allowed is also (50). All surveys must be completed by April 24, 2017 at the close of business. The training materials will be sent directly to your specified address. We encourage you to take advantage of this service and opportunity to standardize our information for prospective families.
