Dear Providers:

The Georgia Division of Family and Children Services engaged Chapin Hall at the University of Chicago to report on the state’s resource development, placement, and resource utilization practices, and to make recommendations for improvement.

As part of the effort to collect information related to the placement process, Chapin Hall would like to hear from the private provider community, and is interested in feedback from individuals who are directly involved in making placement decisions for children/youth in care.

This survey is confidential and may be stopped at any time. It should take approximately 30 minutes to complete, and if you choose, you may take more than one session to do so with the use of the "Save & Return Later” option.
