Dear CPA Providers:

The CPS Screening Request Information form posted on GA+SCORE has been updated to capture the date (in month/year format) for each address in the applicant's history, including the current address. That update aligns it with the memo "CPS application changes for Current/Prospective Foster Parents", sent to you previously and linked below. Please be sure that you are using the updated form when you submit a CPS screening request.

As a reminder, effective 8/1/2017 it is required that all information listed on the application (except for the signature) be typed. The typed information should contain a font size that will be legible to the individual completing the screening request.
