E-Blast: September reminder: CPS Application Process Change

Dear CPA Providers:

Please ensure that you are following the current process for CPS screening requests. Forms should be typed, with no handwritten content except the signature; the signature must be original. The date (in month/year format) must be listed for each address in the applicant's history, including the current address. There should be only one application uploaded for each individual, along with one agency letterhead per family. Requests on older forms, with handwritten content, missing signatures, or with other errors may not be accepted for processing.
