The Office of Provider Management (OPM) is currently in the process of completing contract renewals for FY2019 contract period of July 1, 2018. Your agency’s Certificate of Liability (COL) reflects the insurance coverage presented has expired. If your insurance has or will expire prior to July 1, 2018 please email your COL to Brenda.Jones@dhs.ga.gov or fax to 678-747-6881. Once received, we will begin processing your FY2019 contract.
