The CPS Screening Unit has incorporated provider feedback into three new documents to guide CPAs in preparing requests that can be processed without re-submissions:
- Applicant form (Right click and Save As... for editable PDF)
- Memo on CPS application form change
- Sample completed CPS application form
Please review the expectations for CPS screening requests and begin using the PDF version of the form for all applications. Copies of all documents are posted to www.gascore.com under "CPS Screening".
