E-Blast: FY20 Contract Amendments -- forms included

Greetings Providers,

The goal of the Office of Provider Management (OPM) is to ensure that all FY20 contracts are executed timely and efficiently. This includes all amendments you desire to make within your agency/program (i.e., name change, capacity, address change, program designations…).

All documents for amending your agency’s contract must be submitted to OPM by close of business on January 28, 2019. If not received by that date, amendments will not be processed until the next contracting cycle.

For your convenience, we’ve gathered all applicable vendor request forms:
