Greetings CPA Providers,
Please right click and "Save As" to download the zip file containing all the new SAFE forms (35 MB). The Office of Provider Management (OPM) is requesting that you review all forms in preparation for the upcoming webinar hosted by the Division’s Caregiver Recruitment and Retention Unit (CRRU) via Skype on Wednesday, July 31, 2019 at 10:00am.
**If you have any questions or concerns regarding the forms, please send them ahead of time to ensure that it is addressed during the webinar.**
Prior to the webinar, you will need to download Skype for Business if you do not already have it on your computer. There will not be an option of participating via mobile devices (phones, tablets, etc.) as some components of the webinar do not operate in the mobile app. Please follow the link below to join the session. You will be viewing a PowerPoint presentation and it is recommended that you attempt to access the meeting at least 10 minutes prior to the start time to ensure your equipment works properly.
