Greetings CPA Providers,

The Office of Provider Management (OPM) previously requested a list of all initial foster homes that your agency desired to be processed in the “traditional” format prior to the deployment of the SAFE model on August 24, 2019. This list was requested to be sent via email to opm.leadership@dhs.ga.gov upon completing the initial inquiry information into GA+SCORE for processing. Upon review of a sample, information provided did not capture the pertinent information required to complete this process.   

In an effort to ensure that ALL initial foster homes are captured up to date, OPM is requesting that you complete an Excel spreadsheet for ALL initial foster homes your agency intends to have processed the “traditional” way.   

1. Ensure the homes are listed in GA+SCORE under "Application in Process".
2. Download a copy of the spreadsheet and rename with your provider name replacing "PROVIDERNAME".
3. Enter the required information for each home your agency intends to have processed the "traditional" way.
4. Email the completed sheet to opm.leadership@dhs.ga.gov by Monday, August 12, 2019.
