Greetings ILP providers!
The Office of Provider Management is assisting Communities 4 Children with identifying approximately 320 children/young people currently placed with our Independent Living Providers, who might need holiday support in the way of stuffed stockings this holiday season. If you are interested in receiving donations for the youth in your program, we will need following information on this Excel document:
- First Name and Last Initial of each youth (Please ensure that only the last initial is submitted due to confidentiality purposes)
- Agency Address (please include the address that Communities for Children can physically drop off the donations as well as a mailing address in the event the items have to be mailed due to the pandemic)
- Agency Contact Person (this should be the person that can be contacted to coordinate donation drop-offs)
- Agency Contact's Phone Number
Please submit the attached MS Excel document, with the information specified above, no later than close of business on November 6, 2020. If you have an ILP youth that is a parenting teen, please include their children as well. Communities 4 Children would like to ensure that no child/young person placed at a site is left out!
