Greetings CPA Providers,

The Office of Provider Management (OPM) is in the process of collaborating with our partners to register CPA staff for the upcoming SAFE initial training sessions and SAFE Refreshers. All sessions will be offered virtually and a select number of slots have been allotted for our CPA partners for each session. Therefore, please complete the attached registration based on the criteria for the training listed below no later than November 23, 2020.

Note: The linked documents are fillable PDF forms. You should save them to your computer and open them locally. If you open them inside your browser, you may not have the option to select a session and submit the form.

• SAFE Training Registration Form
  • To attend the 2-day Initial SAFE Training the staff member must be in a position where they will be writing home assessments or approving SAFE assessments.
  • To attend the Supervisor Day for the Initial SAFE Training, you must be providing supervisory oversight to an assessor who is completing a SAFE assessment or you must be an approver of a SAFE assessment.
  • Session dates:
    • December 1-2 (2-Day) –9 am to 4 pm
      December 3 (Supervisor) – 9 am to 1 pm
    • December 8-9 (2-Day) – 11 am to 6 pm
      December 10 (Supervisor) – 11 am to 3 pm
    • December 8-9 (2-Day) – 12 pm to 7 pm
      December 10 (Supervisor) – 12 pm to 4 pm
    • December 8-9 (2-Day) – 10 am to 5 pm
      December 10 (Supervisor) –10 am to 2 pm
  
   
• SAFE Refresher Registration Form 
  • To attend a SAFE Refresher, the staff member must already have the general SAFE Certification.
  • To attend a Supervisor SAFE Refresher, the staff member must already have the Supervisor SAFE Certification.
  • Session dates (All Refreshers are held from 9 am to 4 pm EST):
    • December 16th, 2020
    • January 11th, 2021
    • February 2nd, 2021 (Supervisors Only)
    • February 17th, 2021
    • March 2nd, 2021 (Supervisors Only)
    • March 16th, 2021
    • April 13th, 2021
    • April 27th, 2021 (Supervisors Only)
    • May 18th, 2021
    • June 2nd, 2021
    • June 15th, 2021 (Supervisors Only)
    • June 28th, 2021
