As we approach fiscal year 2023 (FY23) contract cycle, our goal is to ensure that all FY23 contracts are executed timely and efficiently. As you are aware, the Office of Provider Management (OPM) has developed a new process for obtaining and submitting providers' yearly and/or ongoing contract documents via GA+SCORE. For your convenience, we’ve gathered all of the updated documents that will need to be completed for the FY23 contract cycle, including any vendor request changes. (Note that linked file is a 8 MB zip file.)

OPM will need all FY23 contract documents to be completed and uploaded into GA SCORE (DO NOT EMAIL) by close of business on January 12, 2022. To ensure all contracts are executed timely, this deadline cannot be extended. The first file in the zipped folder linked above, "aa FY 23 Contracts Documents Memorandum_December 2021.docx", contains the list of required files.

To add your files in GA+SCORE, select the program and then go to the orange Profile >> Contract Docs screen. Please review the notes at the top of the screen for reminders about restrictions on file size, type, and name. Add new files with the "Upload a File" link. To view files from a previous year, scroll to the bottom, click "Contract Docs History", and select the year. Files from previous years cannot be added or removed.
