Dear Directors:

As we approach the FY17 contract year, our goal is to ensure that all FY17 contracts are executed timely and efficiently. To that end, we will need the required documents submitted to the Office of Provider Management by close of business on January 15, 2016.

Please download the FY17 packet here (right click the link and Save Target As... to download the zipped folder to your computer). The packet includes the Provider Letter (submission instructions and a list of required documents) as well as blank forms.
