OPM has been advised that the State Accounting Office (SAO) has revised the Supplier Change Management Form effective December 2023, and the Internal Revenue Services has issued a new W9 form effective March 2024. The attached memo highlights in yellow the two form changes and the revised RBWO FY25 contract documents. OPM will need the attached revised Supplier Change Management Form and revised W9 form completed and uploaded into GA SCORE (DO NOT EMAIL) by close of business on Friday, April 5, 2024.
