What is required to make a Waiver Request?
You may now use GA+SCORE to submit an online waiver request! At a minimum, you will need information such as the following. Please ensure you have the required components before beginning the submission:
- Contact information for DFCS case manager, supervisor, and county director
- Contact information for RBWO or non-contracted agency, as applicable
- Child's name, Date of Birth, Legal County of Residence
- Summary of Child's Needs/Purpose of Waiver
- Effective Date

If you are requesting a waiver renewal, you must upload a copy of the previous waiver letter. In addition, depending on the type of waiver you are requesting, please have the following available:

What to Expect Once the Request is Submitted Online
CCTU staff will respond to your waiver request using the contact information you provide. Learn more about the response process in the Standard Operating Procedures. You may be contacted via phone or email for to ask additional questions or schedule a staffing to discuss placement options and recommendations on other services that are available.
