To be treated as a Tier 1 (high priority) request, at least one of the following must be currently true:
- Being discharged from a PRTF or CSU
- Currently in a hotel or RYDC
- Substance abuse, sex offender, fire starter, or cruelty to animals
- Chronic runaway (5 or more overnight absences over the last 3 months)
- Commercially Sexually Exploited Children (CSEC)
- Severe behaviorla issues, severe mental health issues, or severe medical conditions (approved for SMFWO program designation)
- Intellectual/Development Disabilities
Please ensure you have the required components before beginning the submission:
- Contact information for DFCS case manager, supervisor, and county director
- Child's name, Date of Birth, Legal County of Residence, and a summary of the child's needs
- One or more uploadable documents relevant to your specific request (.doc, .docx, .pdf, or Open Office formats accepted):
- Universal Application (replaces the MATCH Screening Summary Form) and Frequently Asked Questions
- RBWO Placement MATCH Tracking Form (list of placements contacted and outcomes )
- Psychological or Clinical Assessment
